Engagement of Services
Any actual engagement of BelleMed's credentialing, enrollment, CAQH, or contracting services is governed by a separate services agreement executed directly with your organization. Nothing on this website constitutes an offer to contract on specific terms.
No Guarantee of Outcome
BelleMed coordinates and prepares applications and related work on behalf of clients. Final decisions on credentialing, enrollment, or contracting matters rest with the applicable health plan, government program, or other third party, and are outside BelleMed's control. Approved disclaimer language will be finalized here.
Limitation of Liability
Approved limitation-of-liability terms, applicable to both this website and BelleMed's services, will be added following legal review.
